Provides rehabilitative services to home health and hospice patients. Carries out skills contributing to treatment, rehabilitation, and safety for home health patients and maintenance, education, and safety for hospice patients and families. Plans for coordinated therapy service for individuals and families under her/his care. Appraises individual and family health needs and hazards–existing or potential. Provides clinical duties necessary to provide high quality rehabilitative care. Consults with and refers families to appropriate personnel within the agency or community services. The Home Health/Hospice physical therapist works under the supervision of the supervisor/director in evaluating, planning, implementing, supervising or documenting the care of patients specific to population served with special attention with geriatric population. All the job standards and skills included within this document will be performed and documented according to established policies, procedures, and guidelines within the Home Health/Hospice Department and Trinity Hospital. STANDARDS MEASURED BY: Direct or indirect observation, documentation, valid complaints, QM reports, review and demonstration. Any areas found to be below in a standard for more than 1 consecutive year will result in a below standard rating for the entire standard.

This placement is ideal for undergraduate students, while the following two require more training or experience.

GHANA: Hospital
Africa is more resource poor than India center above and without the diagnostic and intervention tools.
The hospital director would like physical therapists who can treat patients and improve the skills of their staff.
Ghana has earned the reputation as one of the friendliest and “drumming-est” countries in Africa.
You would be about 2 hours from Accra the capital or an hour from Cape Coast. Ghana has a beautiful coast.

UGANDA: Physical Disabilities
Physical (and developmental) disabilities are just beginning to be addressed in Africa.
Many times a disabled person will be rejected by their family, believing they are cursed.
It is to this director’s credit that he has gotten funding to get wheel chairs for people that can not walk.
There is not yet much understanding in the community about disabilities; they would just say someone is “lame”.
They ask for help assessing people that can not walk and then beginning interventions; training staff and community providers; and working with family and community to increase their understanding of disabilities.
This is in a more remote part of northern Uganda and has a fledgling but committed director.
If you like challenges, doing assessment, treatment, and community education, then we’d love your help here.

Experience preferred but NEW GRADS are welcome to apply. You will be allowed 60-minutes for new patients (PTs) and regular treatments are scheduled every 30-minutes. Clinical Assistants will be assigned to you so the volume is high pace but very comfortable and you should enjoy the one-on-one environment.
