We ask that you bring your referral and insurance information to the first appointment and to inform us of any insurance changes throughout the course of your treatment. Co-payments are due at the time of your arrival. We cannot schedule follow-up appointments until all outstanding payments are made. We accept Visa, MasterCard, cash and checks for copay payments.
