Washington University Physical Therapy- Central West End – 4444 is located at 4444 Forest Park Avenue, suite 1210, at the southwest corner of Forest Park and Newstead Avenues. Validated visitor parking is available in the surface lot adjacent to the building. Visitors are required to take a ticket to enter our gated lot. Tickets are validated when patients check in for their appointments. Washington University Physical Therapy – Central West End – @4240 is located on the first floor of the @4240 Heritage building, suite 120, at the corner of Boyle and Duncan Avenues. Free visitor parking is available in the gated surface lot off Duncan Ave., across from the building as well as near the building entrance. Washington University Physical Therapy- O’Fallon is located at 1 Progress Point, near Interstate 40/64 and Highway K in O’Fallon, MO. Same expert care, convenient western location.Washington University Physical Therapy- Central West End- CAM is located at 4921 Parkview Place, Suite 14B, St. Louis, MO. Visitor parking is available in the Euclid Garage, which connects to the 3rd floor of the Center for Advanced Medicine building.Washington University Physical Therapy- Danforth Campus is located in Suite 1051 of the Zetcher House at 6614 Shepley Drive.
